Hockanum Valley Community Council now offers services within our comprehensive Case Management Department. Clients of HVCC and members of the community can access support from one of our skilled and dedicated Case Managers. Our Case Management program is a client-friendly, inclusive program providing assistance to individuals in need.

The goal of our Case Management program is to provide our clients with support for issues that may include but are not limited to: linkage to various State and Federal assistance programs, health care, transportation, basic needs, resume assistance, advocacy, and much more. The entire Case Management Department is committed to offering effective, empathetic services under the guideline principle of LAST: Link, Advocate, Support, Teach.

Case Management services include:

  • Linking to HVCC Internal Services (Counseling, Transportation)
  • Linking to the Tri-Town Food Pantry
  • Linking to Area Agencies
  • Elder Services (support for Senior residents age 60 and over)
  • Support with Applications (Dept. of Social Services, Dept. of Developmental Services, etc)
  • Linking to Prescription Assistance Programs
  • Support with resume writing and job applications
  • Linking to General Assistance Recovery Support Program (GA RSP)
  • Linking to Housing Opportunities (when eligible)
  • Facilitating workshops and recreational groups (Book Club; Nutrition Workshops; Resume Writing, etc)
  • Advocacy

Eligibility:
We serve residents in the towns of Vernon, Ellington, Tolland, and the surrounding area.

Fees:
Case Management services are fully funded by HVCC. There is no cost to clients.

Hours:
Monday 10am - 3pm
Tuesday 2pm - 7pm
Wednesday 11am - 6pm
Thursday 9am - 3pm
Friday 9am - 11am

*Special appointments outside these regular hours may be made in the event of an emergency.

To make an appointment with Case Management, you may call anytime during normal Counseling hours, Mon-Thurs 9:00am-8:30pm and Fri 9:00am-1:00pm.
