HVCC is excited to announce the development of our new comprehensive Case Management program. Clients of HVCC and members of the community can now access support from one of our skilled and dedicated Case Managers. In addition to our two new Case Managers, the department has also added a Community Organizer/Educational Coordinator.
Our Case Managers will also be directly serving our senior clients with programs within our Elder Services division, including medical, financial, and social services. You will notice that our Elder Services web page now directs to the new Case Management page; however all the previous services which we offered to our senior clients are still available through this new program.
Our Transportation department is currently contracted with Eastern Connecticut Health Network to run 4 full-time courier routes. Our drivers transport medical specimens, lab tests, x-rays, and paperwork between local doctors' offices and hospitals in Bloomfield, South Windsor, Manchester, Tolland, Coventry, Ellington, and Vernon. This is just another service we are proud to offer our community!
